The Family Resource Center for Disabilities and Special Needs and the Parent Training and Resource Center (FRC/PTRC) are committed to safeguarding the personal information entrusted to us by our clients. We obtain information directly from our clients or by means of a signed release of information. We manage personal information with confidentiality and do not share information outside our agency unless permission is provided. This applies to all staff and board members within the FRC/PTRC and to those persons providing services on our behalf.
Personal information refers to information about an identifiable individual, including children. This includes an individual’s name, home address, phone number, age, sex, race, educational history, military service, disabling condition, etc. We collect only the personal information that we need for the purposes of providing services to our clients, including personal information needed to:
• Enroll a client for services
• Deliver requested services
• Send out FRC/PTRC information
• Provide information required by our funders.
If personal information is used in mandatory reporting to funders, the information is not associated with a specific individual. For example, a funder may request the number of parents of children with autism we serve during a given year. The names of these parents, nor their children, are not included with the data provided.
We normally collect client personal information directly from our clients. However, we may collect information and/or records from other persons with written consent or as authorized by law. We protect personal information in a manner appropriate for the sensitivity of the information. We make every reasonable effort to prevent any loss, misuse, disclosure, as well as any unauthorized access to personal information. We retain personal information only as long as is reasonable to fulfill the purposes for which the information was collected. We use appropriate security measures when destroying personal information, including shredding paper records and permanently deleting electronic records.
